At Liberty Home and Community Care, we provide personalized, one-on-one support to help individuals remain safe, comfortable, and independent at home and in their community.
We proudly serve individuals enrolled in the Connecticut Home Care Program for Elders (CHCPE), as well as those seeking private pay services. Care can be arranged full-time, part-time, or as needed, depending on each person’s goals and level of support.
Our services are delivered through compassionate 1:1 care, tailored to meet individual needs.
Companion Services:
Companion services focus on social support, engagement, and assistance with everyday activities that promote independence and emotional well-being.
Companion services may include:
Friendly conversation and companionship
Emotional support and monitoring
Accompaniment to appointments and community activities
Assistance with errands and shopping
Medication reminders (non-medical)
Recreational activities and hobbies
Safety monitoring and wellness checks
These services help reduce isolation while encouraging meaningful daily routines.
Homemaker Services
Homemaker services help maintain a clean, safe, and organized living environment so individuals can continue living comfortably at home.
Homemaker services may include:
Light housekeeping and household organization
Laundry and linen changes
Meal planning and meal preparation
Grocery shopping and household errands
Maintaining a safe home environment

Liberty provides both private pay services and care funded through waiver programs. These programs may help eligible individuals receive support services at home and in their community at little or no cost.
